Algemene opmerkingen

  • Geen bewezen effectieve therapie in afwezigheid van immuun herstel.  Primare behandeling is herstel van immuniteit (in geval van HIV/aids; antiretrovirale therapie).
  • Duur therapeutische behandeling op geleide van klinisch beeld EN eliminatie van oöcysten uit ontlasting EN CD4 > 200/µL.
  • De effectiviteit van antiparasitaire behandeling is in het algemeen beperkt (niet-curatief) en kan minimaal zijn in ernstig immuungecompromitteerde patiënten (Pantenburg B et al. Expert Rev Anti Infect Ther 2009;7:385-391)
  • Bij ernstige klachten kan agressieve orale en iv ondersteunende therapie (rehydratie/electroliet suppletie) nodig zijn, in combinatie met anti-diarree middelen (opium tinctuur lijkt het meest effectief).

Bij HIV:

  • Immuunreconstitutie door antiretrovirale therapie leidt tot complete eliminatie van de parasiet (Bachur TP et al. Braz J Infect Dis 2008;12:115-22).
  • Een protease-inhibitor (PI) gebaseerd cART regime valt te overwegen aangezien in vivo data laat zien dat PI de replicatie van Cryptosporidium species remt (Mele et al. Int J Parasitol. 2003;33:757-64).

Paromomycine

  • is in Nederland niet geregistreerd maar is een zogeheten "ZFW-middel", het wordt vergoed voor patiënten met HIV-infectie en cryptosporidiose, in andere gevallen niet.

Nitazoxanide

  • innemen tijdens of kort na een maaltijd.
  • is in Nederland niet geregistreerd maar is een zogeheten "ZFW-middel".
