Algemene opmerkingen
Een positieve cathetertip (arterieel of veneus) bij een patient met negatieve bloedkweken vereist over het algemeen geen antibiotische therapie. Er zijn echter enkele uitzonderingen:
- Staphylococcus aureus op tip: Neem bloedkweek af (ook indien patient koortsvrij is) en start met flucloxacilline iv 2000mg 6dd. Wanneer bloedkweken 72u later nog negatief zijn, kan geswitcht worden naar een oraal regime (in overleg met consulent infectieziekten) en volstaat een totale behandelduur van 7 dagen.
- Gist/candida spp. op tip: Neem bloedkweek af in specifiek gist/schimmel (mycose) flesje en start indien patient niet ernstig ziek is en niet is voorbehandeld met een azol: fluconazol po/iv 400mg 1dd (dag 1 opladen met 800 mg). Op basis van candida species determinatie en gevoeligheid zo nodig behandeling aanpassen. Bij azol (fluconazol, itraconazol, posaconazol, voriconazol) voorbehandelde patient altijd overleg met consulent infectieziekten. Wanneer bloedkweken na 5 dagen nog negatief zijn, kan de antifungale therapie gestaakt worden.
