General comments
Methicilline gevoelig: beleid geldt bij methicilline gevoelige Staphylococcus aureus (MSSA) en methicilline gevoelige coagulase-negatieve stafylokokken (MSSE)
Dosering vancomycine iv bij normale nierfunctie: 1000 mg 2 dd (<70kg) of 1000 mg 3 dd (>70kg) met een eerste vancomycine dalspiegel op dag 2.
Doseer vancomycine voor dalspiegels van 15-20 mg/l.
Toevoegen rifampicine alleen indien Staphylokok rifampicine gevoelig is.
Rifampicine pas starten na 3-5 dagen EN nadat bloedkweken negatief zijn geworden om snelle resistentievorming te voorkomen.
