Renal function-replacement therapy

CAPD: IV: oplaaddosis 20 mg/kg (max. 2000 mg), gevolgd door 15 mg/kg elke 4-7 dagen; intraperitoneaal diverse schema’s bijv. 2 g ip 1x/5-7 dagen of 1 dd 50 mg via CAPD zak voorafgegaan door een oplaaddosis.

Hemo dialysis: <70 kg: 1000 mg oplaaddosis gevolgd door 500 mg onderhoudsdosis. 70-100 kg: 1250 mg oplaadosis gevolgd door 750 mg onderhoudsdosis. >100 kg: 1500 mg oplaaddosis gevolgd door 1000 mg onderhoudsdosis.

CAV / VVHD: IV: oplaaddosis 20 mg/kg (max. 2000 mg), gevolgd door 1000 mg continue per 24 uur, verder op geleide spiegels
