General comments

Of er sprake is van 'ernstige' danwel 'niet-ernstige' neutropene koorts is een klinische inschatting/beslissing.

Bij patiënten met neutropene koorts en een laag risico op een gecompliceerd beloop kan overwogen worden om na afname van (bloed)kweken te starten met orale antibiotica (amoxicilline-clavulaanzuur po 625mg 3dd plus ciprofloxacine po 500mg 2dd). Na 72 uur wordt op basis van het klinische beloop en de kweekuitslagen besloten om de antibiotica te stoppen of de behandeling te stroomlijnen. 

Criteria voor thuisbehandeling:

  • MASCC score ≥ 21
  • Geen PTC drain, geen PCN drain
  • Geen galwegpathologie
  • Geen centrale lijn of PICC-lijn
  • Niet bekend met micro-organismen die resistent zijn tegen amoxicilline-clavulaanzuur en/of ciprofloxacine 
  • Geen allergie voor beta-lactam antibiotica en/of quinolonen
