Indications: Penicillineallergie of amoxicilline resistent en high level gentamicine resistent (MIC>128 mg/L)

	Medication: Vancomycine iv 1000mg 2dd 6 weeks

Dosering vancomycine iv bij normale nierfunctie: 1000 mg 2 dd (<70kg) of 1000 mg 3 dd (>70kg) met een eerste vancomycine dalspiegel op dag 2.

Doseer vancomycine voor dalspiegels van 15-20 mg/L.
