Indications: Penicilline MIC kleiner of gelijk aan 0.125mg/L

	Medication: ceftriaxon iv 2000mg 1dd 6 weeks	Remarks: Bij allergie (m.u.v ernstige, niet acute allergie)
	Medication: Vancomycine iv 1000mg 2dd 6 weeks	Remarks: Bij ernstige, niet acute allergie (bv. SJS, TEN)

≥ 18 years

Indications: Penicilline MIC van 0.250 - 2mg/L

	Medication: benzylpenicilline iv 3million. IU 6dd 6 weeks + gentamicine iv 3mg/kg 1dd 2 weeks
	Medication: ceftriaxon iv 2000mg 1dd 6 weeks + gentamicine iv 3mg/kg 1dd 2 weeks	Remarks: Bij allergie (m.u.v ernstige, niet acute allergie)
	Medication: Vancomycine iv 1000mg 2dd 6 weeks	Remarks: Bij ernstige, niet acute allergie (bv. SJS, TEN)

≥ 18 years

Indications: Penicilline MIC groter dan 2mg/L

		Remarks: Behandeling in overleg met de consulent infectieziekten

Dosering vancomycine iv bij normale nierfunctie: 1000 mg 2 dd (<70kg) of 1000 mg 3 dd (>70kg) met een eerste vancomycine dalspiegel op dag 2.

Doseer vancomycine voor dalspiegels van 15-20 mg/l.
