Oroya fever

Bij ernstig ziek zijn en/of persisterende ziekte: ceftriaxon toevoegen
Medication:

azitromycine po 500mg 1dd 7 days

or

ciprofloxacine po 500mg 2dd 7 to 10 days
Remarks:

B. bacilliformis (Zuid-Amerkiaanse bartonellose/Carrion's disease)

Verruga peruana

Eerste keus azitromycine, tweede keus ciprofloxacin
Medication:

doxycycline po 100mg 2dd 4 weeks

+

gentamicine iv 3mg/kg 1dd 14 days
Remarks:

B. quintana (loopgravenkoorts)

Indien bacteriëmie zonder endocarditis.

 

 
Medication:

azitromycine po 500mg one-off

followed by

azitromycine po 250mg 1dd 4 days
Remarks:

B. henselae (kattenkrabziekte)

Tweede keus: ciprofloxacin 2 dd 500 mg of claritromycine 2 dd 500 mg of cotrimoxazol 2 dd 960 mg gedurende 7-10 dagen.

Bij CNS- of orgaanlokalisatie: overleg met de consulent infectieziekten.

 

General comments

Bij gebruik van gentamicine is spiegelcontrole aangewezen.

Bij HIV infectie en bacillaire angiomatose (B. quintana en B. henselae) of orgaanlokalisatie: doxycycline po 100 mg 2 dd gedurende 3 maanden

Bij HIV infectie en CNS-lokalisatie en/of levensbedreigende ziekte: overweeg de toevoeging van rifampicine po 300 mg 2 dd

Bij HIV infectie en bacteriëmie, behandelen als endocarditis.
