General comments

  • Overleg met consulent infectieziekten

  • Overweeg bij immunodeficiente gastheer combinatie therapie te starten: liposomaal amfotericine B iv, totale dosis  van 30mg/kg, 6 x 5mg op alternerende dagen, plus miltefosine 5 mg 2dd gedurende 28 dagen

  • Voor een uitgebreider overzicht therapie per Leishmania species, kan het artikel van Hodiamont et al geraadpleegd worden en "Antiparasitic Treatment Recommendations" van Andreas Neumayr (zie bronnen).
  • Secundaire profylaxe en/of monitoring is aangewezen bij immuungecompromitteerde patienten (bij HIV en aantal CD4+ T-cellen < 350/mm3)

    • Monitoren m.b.v. moleculaire diagnostiek op volbloed en indien weer positief, hervatten behandeling
    • Secundaire profylaxe: maandelijks miltefosine, pentamidine of pentavalent antimoon preparaat
