Praziquantel behandeling op dag 3 na start prednisolon en herhalen na 3 maanden

General comments

Acute schistosomiasis (Katayama syndroom):

  • praziquantel is niet/weinig effectief tegen larvale stadia.
  • afhankelijk van klinische verschijnselen, eerst behandelen met corticosteroïden (prednisolon po, 1 dd 40 mg, 3-6 dagen) en daarna met praziquantel behandelen als adulte wormen ontwikkeld zijn (ca. 6-8 weken na infectie).
  • bij ernstige klachten tijdens acute fase (Katayama fever) overweeg naast prednisolon behandeling met riamet (mogelijk gunstig effect). Dosering zie behandeling malaria tropica (Plasmodium falciparum). 

Eenmalige orale behandeling met praziquantel bij reizigers resulteert in falen van de behandeling (ca. 20-30%). Herhaalde behandeling na 4 weken is daarom aanbevolen. Zie bijvoorbeeld volgende publicaties:

Praziquantel

  • Innemen tijdens of kort na een maaltijd.
  • Onvoldoende bekend over veiligheid tijdens zwangerschap

Neurale schistosomiasis (neuroschistosomiasis)

  • Bij myelitis transversa
    • Methylprednisolon 15mg/kg/dag (max. 1 g) voor 5 dagen, gevolgd door prednisolon 1 mg/kg/dag voor 6 maanden (na ca. 4 weken afbouwen op klinisch geleide)

    • Praziquantel p.o. 2dd 30 mg/kg voor 3 dagen en herhalen na 4 weken
  • Bij cerebrale schistosomiasis
    • prednisolon 1 mg/kg/dag voor 2 maanden (na weken afbouwen op klinisch geleide)
    • +
    • Praziquantel p.o. 2dd 30 mg/kg voor 3 dagen en herhalen na 4 weken 

Behandeling voor Schistosoma mekongi is gelijk aan die van S. japonicum

Behandeling voor Schistosoma intercalatum is gelijk aan die van S. mansoni en S. haematobium

Bij persisterend therapie falen van praziquantel, overweeg artesunate–mefloquine in anti-malaria dosering (artesunate 4 mg/kg + mefloquine 8 mg/kg p.o. , 1dd voor 3 dagen) (zie Bottieau et al 2024).
