General comments
Behandeling is altijd multidisciplinair van aard en altijd in overleg met de (tropen)consulent infectieziekten.
Chirurgische therapie is waarschijnlijk de enige curatieve therapie bij alveolaire echinococcose en is eerste keuze behandeling. Preoperatief behandelen met albendazol gedurende 28 dagen (ook bij levertransplantatie). Medicamenteuze behandeling is na chirurgische behandeling noodzakelijk bij alle patiënten en is tijdelijk in het geval van radicale resectie (minimaal twee jaar) en in de regel levenslang voor alle andere patiënten.
Indien chirurgie niet mogelijk dan therapie met antihelminthica. Duur van behandeling onduidelijk, minimaal 2 jaar en mogelijk levenslang.
- niet tijdens zwangerschap en lactatie.
- tijdens zwangerschap alleen op strikte indicatie (bij dieren is therapeutische dosering schadelijk gebleken)
- geregistreerd voor kinderen vanaf 6 jaar (tussen 1-6 jaar kan gebruik overwogen worden, Keiser et al).
- innemen met vetrijke maaltijd (verhoogt de resorptie).
- bij langdurig gebruik, controleer leverfuncties en leukocytenaantal.
