General comments

  • Er is geen therapie bekend die tot genezing van een oculaire Toxoplasma-infectie leidt. De behandeling is er op gericht om de schade in het oog en de slechtziendheid en blindheid te beperken. Behandeling is alleen geïndiceerd wanneer de visus door de lokalisatie van de laesie op de retina bedreigd wordt. Behandeling met pyrimethamine, sulfonamiden kan de tachyzoïeten (die tijdens de acute fase van de infectie voorkomen) doden, maar de bradyzoïeten in de weefselcysten blijven vitaal.
  • Wegens gebrek aan goede, gerandomiseerde studies is de literatuur ook niet eenduidig over beste behandel methode. De combinatie pyrimethamine met sulfadiazine wordt het meest gebruikt en lijkt het meest effectief.
  • Indien macula betrokken dan corticosteroiden bijgeven
  • Bij ernstige oculaire toxoplasmose: overweeg quadriple therapie (pyrimethamine + sulfadiazine + clindamycine + folinezuur)
  • Kleine perifere laesies in immuuncompetente patiënten niet behandelen (zelf-limiterend).
  • Reactivatie van een occulaire toxoplasmose tijdens de zwangerschap geeft geen verhoogd risico op congenitale toxoplasmose. Bij reactivatie van oculaire toxoplasmose tijdens de zwangerschap is pyrimethamine gecontraindiceerd, waardoor mono-therapie met clindamycine1e keuze is. Mono-therapie clindamycine onderdrukt actieve infectie, maar kent hoog percentage recidief na behandeling.
  • Kans op reactivatie van ernstige oculaire toxoplasmose lijkt te verminderen door profylaxe na therapie (zie Silveira et al 2002): cotrimoxazole po (1 dd 960mg per 3 dagen voor 20 maanden)
  • Als alternatieve behandeling kan ook gekozen worden voor intravitreale injecties met clindamycine (zie Soheilian et al. 2011 en Verma et al 2020)
