General comments
In principe niet behandelen tenzij er geen andere oorzaken voor de (langdurige) buik- en/of diarreeklachten zijn vastgesteld.
Asymptomatisch dragerschap komt frequent voor.
Geen eenduidige literatuur t.a.v. 1e keuze.
Clioquinol:
- verkrijgbaar als suspensie LNA (100 mg/ml)
- gebruik tijdens de zwangerschap en lactatie wordt afgeraden. Clioquinol bevat jodium die opgenomen kan worden door de foetale schildklier. Ook kan dit de schildklierfunctie van de zuigeling tijdens lactatie beïnvloeden.
- Bij een (cumulatieve) dosis van 10–50 gram clioquinol is er meer kans op neurotoxiciteit waardoor aangeraden wordt om clioquinol niet hoog te doseren in een korte periode. Bij herhaalde behandeling wordt 3 kuren per jaar met enkele maanden ertussen, aanbevolen.
Paromomycine is in Nederland niet geregistreerd maar is een zogeheten "ZFW-middel" en wordt vergoed voor patiënten met HIV-infectie en cryptosporidiose; in andere gevallen niet.
Metronidazol is inferieur aan clioquinol en paromomycine. Metronidazol dient niet tegelijkertijd met een alcoholische drank te worden genuttigd ivm het zg. antabuseffect
