General comments
Overleg met de consulent infectieziekten:
1. Mbt. in te zetten diagnostiek (serologie en PCR) naar niet kweekbare verwekkers, o.a. Coxiella burnetii (Q-koorts) en Bartonella. Zie hiervoor ook 2020 DEF_Addendum Endocarditis.pdf (nvvc.nl). Denk daarnaast aan streptococcen PCR.
2. Mbt. antibioticabeleid. Beleid bij kweek negatieve endocarditis is mede afhankelijk van eerder antibioticagebruik en resultaten van bloedkweken (indien afgenomen voor start antibiotica). Pas desgewenst het beleid daarop aan.
* Indien verwekker is aangetoond, richt therapie op aangetoonde verwekker
* Stop doxycycline als diagnostiek naar niet kweekbare verwekkers negatief is
