	Medication: ceftriaxon iv 2000mg 1dd + Vancomycine iv 1000mg 2dd	Remarks: Bij allergie (m.u.v ernstige, niet acute allergie)
	Medication: Vancomycine iv 1000mg 2dd	Remarks: Bij ernstige, niet acute allergie (bv. SJS, TEN)

≥ 18 years

Indications: Acuut en fulminant beloop of intraveneus drugsgebruik

	Medication: flucloxacilline iv 2000mg 6dd
	Medication: cefazoline iv 2000mg 3dd	Remarks: Bij allergie (m.u.v ernstige, niet acute allergie)
	Medication: Vancomycine iv 1000mg 2dd	Remarks: Bij ernstige, niet acute allergie (bv. SJS, TEN)

Dosering vancomycine iv bij normale nierfunctie: 1000 mg 2 dd (<70kg) of 1000 mg 3 dd (>70kg) met een eerste vancomycine dalspiegel op dag 2.

Doseer vancomycine voor dalspiegels van 15-20 mg/l.
