Dosis aanpassing bij nierfunctiestoornis (van toepassing bij dosering van 1500mg 3dd iv):
-bij klaring >=30 ml/min: 3dd 1500 mg
-bij klaring >=10 en < 30 ml/min: 2dd 1500 mg
-bij klaring <10 ml/min: 1dd 1500 mg
-bij CVVH: opladen met 3dd 1500 mg gedurende 24 uur, daarna verlagen naar 2dd 1500mg. Bij ernstige infecties handhaven op 3dd1500 mg
